Outreach Services

Outreach services connect individuals living in community settings, who are 60 years or older, minority and/or disabled, to service providers, community health systems and programs that will help them maintain their independence for as long as appropriate.

The program identifies and resolves issues unique to the older population, specifically those related to transportation, housing, moving/hauling, home health need, equipment and medication acquisition. Other services include assistance with medical applications, bill paying, house cleaning, grocery shopping, chore services, speaking with case managers, personal care and legal issues.

If you are an older person living in zip codes 43201, 42303, 43205, 43206, 43207, 43211, 43215 and 43219 we can help you find the assistance you need to meet your needs.

Funding for the program is made possible through Franklin County Senior Options Levy.
